Position Description: 

In the United States, homicides are the third leading cause of death among all individuals aged 15-34 years and the leading cause of death for African Americans in the same age group. Nationally, up to 35% of those violently injured are re-injured within a year. This makes a violent injury one of the strongest risk factors for a future violent injury. The San Francisco Wraparound Project (Wraparound), housed in Zuckerberg San Francisco General Hospital (ZSFG), was designed to break this cycle of violent injury.

Since 2005, Wraparound has leveraged its unique access to individuals at high risk of re-injury to connect with victims at the time of their injury when they are most amenable to services. After initial stabilization, victims of violent injury between the ages of 10 and 30 years are screened by Wraparound’s culturally competent case managers for inclusion in the program. Wraparound clients receive intensive, individualized case management services and are guided through transformative risk reduction − vocational and educational resources to address their underlying risk factors and reduce their risk of re-injury.

The majority of Wraparound clients belong to minority populations (over 80% are African American or Latino) and following injury, face unemployment, mental health disorders, legal issues, and serial trauma. To address this, Wraparound maintains a large assortment of programs to ensure that all clients receive culturally acceptable interventions to address their specific needs. Wraparound has been shown to be both effective and cost-effective. Since Wraparound’s inception re-injury rates in ZSFG have reduced by 50%.

The Wraparound Project is seeking to host a recent graduate for an Equal Justice Works fellowship. We hope to extend our portfolio of services to include in-house assistance with clients’ legal issues. A significant portion of Wraparound’s clients have interacted with both civil and criminal justice systems and require legal services. A majority of clients require assistance with criminal or immigration issues; however, other legal needs identified include housing, family law, and benefit maintenance. The fellow will serve as a liaison between Wraparound clients and their full-time legal service providers. The fellow will work very closely with Wraparound’s case managers, sitting in on weekly case management meetings where Wraparound clients are discussed in order to identify potential legal issues. The fellow will then help case managers refer clients as appropriate, informing clients of what to expect from their legal service providers and ensuring that clients understand every step of the process. The fellow will also participate in the development of client care plans in order to identify potentially missed legal issues.

This position will be based in ZSFG and will require the fellow to be on-site at least twice a week. The successful fellow will be on-boarded as a ZSFG volunteer and will be required to undertake HIPAA training. The fellow might also be required to receive vaccinations as outlined by the hospital.
